Southern Cross Care (NSW & ACT) invites applications for a full-time Clinical Care Coordinator at our Cootamundra facility.
You will support Registered Nurses and care staff in clinical management, maintain high standards of care, and promote a positive work setting.
Responsibilities include monitoring residents' care plans, advising on complex nursing needs, driving continuous improvement, updating documentation, and liaising with doctors, hospitals and other health professionals.
