Monash Health is seeking an Aboriginal Liaison Officer to join the Aboriginal Health Integrated Care team.
The role delivers culturally secure, patient-centered care to Aboriginal and Torres Strait Islander patients across hospital and community settings.
You will work within AHIC, supporting early intervention, inpatient coordination, discharge planning and post-discharge follow-up, acting as a cultural broker to ensure care is clinically effective and culturally safe.
