War Memorial Hospital, operated by Uniting NSW.ACT, is seeking an experienced RN to join the Transitional Aged Care (TAC) team.
The role delivers nursing and case management interventions for older clients within a goal-oriented program, aligned with NSW Health and SESLHD policies.
You will plan and coordinate care, participate in quality improvement, and support a multidisciplinary team in a community-oriented TAC setting in Sydney's Eastern Suburbs.
