Yorke and Northern Local Health Network in Port Broughton is seeking an Enrolled Nurse to coordinate the Transition Care Program (TCP). You will assess client suitability, coordinate admissions, and set individual goals to support independence and secure return home.

Working with patients, families and the care team, you will coordinate allied health services during admission and prepare for discharge, under the Nursing Unit Manager.
