Position Summary:

We are seeking a passionate and clinically skilled Registered Nurse (RN) to join our team. This role plays a vital part in helping older people live safely, independently, and with dignity at home. The RN will lead clinical assessment, care planning, risk management, and clinical governance activities, ensuring safe, evidence-based, and person-centred care. Working closely with participants, carers, and multidisciplinary teams, you will support wellness, reablement, and sustainable care outcomes, particularly for those with high or complex needs.

Main Duties and Responsibilities:

Conduct comprehensive clinical assessments and develop care plans aligned with participant needs and goals;

Provide clinical oversight and guidance to Care Partners and support workers;

Monitor, review, and manage risks for participants, including those with complex needs;

Lead or contribute to case conferences, ensuring clinical recommendations inform care decisions;

Take on the role of clinical care partner for Restorative Care Pathway;

Deliver pathway-specific nursing input across End-of-Life,



and Assistive Technology & Home Modifications (AT-HM) pathways;

Provide education to participants and carers on health management; and

Maintain accurate clinical documentation and contribute to quality improvement and clinical governance activities.

Selection Criteria:

Current registration with the Nursing and Midwifery Board of Australia as a Registered Nurse;

Clinical problem-solving skills and a commitment to delivering quality person-centred care;

Good spoken and written English, fluency in spoken Cantonese;

Good communication, interpersonal, time management and computer skills;

Solid understanding of the needs of people from culturally and linguistically diverse backgrounds;

Possess valid National Police Certificate or is able to obtain the certificate prior to employment;

Possess valid driver licence and own vehicle; and

Current First Aid certificate.

Experience in Government funded home aged care programs; and

Fluency in spoken Mandarin.

Salary and employment conditions for the above position is as per Enterprise Agreement. Salary packaging is available. Discount child care fees are available for CASS's long day care, before and after school, and vacation care services. Employees are also eligible for the Community Services Industry Portable Long Service Leave Scheme.
