Darling Downs Health in Toowoomba is seeking a Clinical Nurse Consultant – Frailty Intervention Community Service to lead advanced assessment, care planning and interprofessional collaboration for frail older people living in the community.

You will drive service improvements, provide clinical leadership, and mentor staff while partnering with patients and families to shape safer, person-centred care across our region.
