BlueCare is seeking a Clinical Care Partner to oversee and coordinate complex clinical and care needs in community settings across Arundel and Southport areas. You will develop and review care plans with clients and the multidisciplinary team, manage budgets, and liaise with clinical teams to respond to changing needs.

You’ll travel to meet clients and support high-quality, person-centred care. Applicants must be Registered Nurse or Allied Health with AHPRA, hold a current Australian driver's
