H1 Healthcare in Sydney is seeking an experienced Care Coordinator to join our Sydney team. You will coordinate disability and in-home support for clients across NSW, with a focus on Icare-funded participants.

You’ll juggle priorities, liaise with families and case managers, and ensure clients receive care at home and in the community. Ideal candidates bring robust Icare program experience, disability coordination skills, and the ability to manage rostering and reporting.
