NSW Health seeks a Health Clinician to join the Community Mental Health team across Western NSW. The role delivers comprehensive case management and care coordination for adults with acute and complex mental health challenges, within a multidisciplinary team.

You will conduct assessments, interventions, care planning and advocacy, partnering with consumers, families, carers and service providers to support recovery goals.
