Dolphin Wellness Care in Sydney, NSW is seeking a Community Services Coordinator to coordinate SIL, daily living and in-home aged care supports. This full-time role focuses on assessments, service plans and rostering to ensure protected, goal‑oriented outcomes for clients.

You will liaise with clients, families and health professionals, monitor progress and ensure compliance with NDIS and aged care standards. A permanent FT position with salary AUD 85,000–92,000 plus super.
