South Western Sydney Local Health District is recruiting an experienced Allied Health Leader to oversee the Transitional Aged Care Program team. You will manage case coordination, workforce planning and governance, ensuring high quality, secure care for older adults transitioning from hospital to community settings.

You will collaborate with nurses, OTs, PTs, social workers and other clinicians to maximise outcomes, while aligning with NSW Health standards and policy.
