As we continue to grow rapidly, we remain committed to empowering individuals to live safely, independently, and with dignity. /n About the Role /n The Community Field Nurse is a front-line position acting as a representative of My Guardian providing the services offered by the company. Their duties are not limited to those listed as the position has a wide scope of duties at times dependent on the needs of the company. /n This position suits someone who enjoys structure, works well under pressure, and thrives in a fast-paced care environment. /n Key Responsibilities /n /n

- Conduct comprehensive home assessments and design personalised care plans that truly meet each client’s needs.

/n

- Lead, mentor and empower care teams, providing expert nursing guidance and ensuring seamless service delivery.

/n

- Deliver hands-on clinical care, including wound management, medication administration and dementia assessments.

/n

- Monitor client wellbeing, promptly identifying and reporting any changes in health or behaviour.

/n

- Coordinate holistic, multidisciplinary care by liaising with hospitals, doctors and allied health professionals.

/n





- Maintain accurate, compliant documentation aligned with policy, funding and regulatory standards.

/n

- Build strong client relationships while fostering a high-performing, customer-focused culture.

/n

- Support operational excellence through collaboration with My Aged Care, NDIS and internal administration teams.

/n /n About You /n /n

- Current and valid AHPRA Registration

/n

- Ability to work independently

/n

- Excellent communication and listening skills

/n

- Compassionate, patient and caring nature

/n

- High level of organisational and time management skills

/n

- Competent using Google Docs and able to learn business systems quickly

/n

- Must have experience with care systems.

/n

- Sound computer skills in Office and Excel (or similar)

/n

- Able to work in a fast-paced workplace.

/n

- Driver’s license with access to own vehicle
