Clinical Care Manager - Community Aged Care

n A well-established Brisbane community provider is now seeking an experienced RN to step into an exciting Clinical Care Manager role. This position offers a strong mix of clinical oversight, team support and service development within a cooperative, forward-thinking environment.

n You’ll play a key role in providing high quality clinical practice and enhancing client outcomes, with a great balance of office and field work. Key Duties:n
n

n
- Conducting client assessments, care planning and ongoing reviews
n




- Managing Support at Home package budgets and funding utilisation
n
- Coordinating services across support workers, nursing and allied health
n
- Identifying changes in client care needs and arranging appropriate support
n
- Building strong relationships with clients, families and healthcare professionals
n
- Maintaining accurate client records and documentation
n
- Ensuring high-quality care in line with the Aged Care Quality Standards
n

About You:n
n

n
- Current AHPRA registration (RN, EN or Allied Health)
n
- Previous experience in community aged care and case management
n
- Strong understanding of Support at Home funding, care planning and budgets
n
- Confident managing an individual client caseload
n
- Experience supporting clients with complex care needs
n
- Strong communication and relationship-building skills
n
- Organised, proactive and confident using client management systems
n
