We deliver high quality, evidence-informed NDIS Support Coordination (Level 2 and Specialist Level 3) and Psychosocial Recovery Coaching, as well as capacity-building Social Work interventions that promote wellbeing, safety, and social inclusion. About the Role: Based in Perth, WA and offered as a hybrid position, combining in office work but majority is work-from-home. Day-to-day responsibilities may include developing and implementing support plans,



and providing ongoing case management and advocacy. The role typically involves coordinating with community services, healthcare providers, and government agencies, as well as maintaining accurate, confidential records and documentation.

We are looking for a number of team members who are seeking either casual/part time or sub contract work (preferred), who are seeking a minimum of 3 days a week work and will be paid for a percentage of billable hour. Applicants must be committed to embracing the team culture which already exists via participation in team meetings, supervision, and service planning. Applicants will build up caseloads and be responsible for compliance of their clients, however support is offered to assist with this and other aspects of the role.

Fluency in verbal and written English language is required, however we welcome multi-lingual CALD applicants. Mobile phone provided, but you will need your driver's license, a reliable car and computer/laptop to fulfil your duties.
