Key Responsibilities :
Provide patient-centred care including immunisations, wound care, assisting in procedures and chronic disease management.
Assist GPs with care plans, health assessments, recalls, and patient education.
Maintain cold chain, infection control, and accreditation standards.
Take initiative, identify patients for preventive care and support quality improvement.
An active role in preparing and maintaining documentation required for practice accreditation and compliance
Extract and analyse patient population data using POLAR and clinical software systems.
Identify patient cohorts eligible for preventative health initiatives, chronic disease management programs, and targeted health clinics.
Work collaboratively with GPs and the healthcare team to improve preventative care outcomes and quality improvement activities.

About You :
Current AHPRA registration (Registered Nurse)
Current CPR certificate
Immunisation certification (or willingness to complete within 3 months)
Past experience in general practice nursing
Reliable and proactive with a positive attitude
Critical thinker and problem solver
