About the role

Evolving Care Advanced is a nurse-led community health service delivering holistic, person-centred care in people's homes. We support clients through NDIS, aged care and private pathways, including in-home support and post-hospital discharge care. Our tiered community nursing framework and robust clinical governance mean you're never left guessing and never left alone.

Key responsibilities
Wound care, medication management, continence and catheter care, diabetes management and post-discharge monitoring, all within your scope and with supportive RN clinical oversight
Delivering delegated care and escalating changes with confidence
Supporting NDIS support workers
Working with families, GPs and allied health so no one falls through the gaps
Helping build a patient safety culture that means something

About you
Current AHPRA registration as an Enrolled Nurse, without conditions that restrict community practice
Blue Card – Working with Children
Yellow Card - NDIS Worker Screening Clearance
Evidence of immunisation status [Hepatitis B, MMR, pertussis, varicella]
Current police check




Current driver's licence and reliable, insured vehicle
Current CPR and First Aid (nationally recognised certificate)
Strong communication skills and a genuine commitment to person-centred care
Desirable: Wound management experience
Desirable: Community, disability, aged care or chronic disease experience
Desirable: Experience with post-hospital discharge care
