You'll assist a group of clients with various disabilities and daily support needs, who live together in a shared home.
You'll be a key part of helping them achieve greater independence and wellbeing in their daily lives.
Operating under the active support model, the role involves support with all activities of daily living including psychosocial support, physical support, support maintaining their household, support maintaining health and wellbeing.Key responsibilitiesDelivering active support aligned with each client's goals and support plansSupporting with daily living activities in their home including cooking and cleaningAssisting with daily personal care tasks (e.g., toileting, showering, dressing)Using protected manual handling techniques in accordance with individual plansFacilitating community access and transport using Unisson vehicles or public transportBuilding trusted relationships with clients, families and external stakeholdersMaintaining accurate documentation, reports and recordsWorking collaboratively within a close-knit teamResponding appropriately to incidents in line with Unisson and NDIS guidelinesAbout youWilling to work active night & sleepover shifts, weekends, and public holidaysHold a valid NSW Driver's Licence (P2/full) and are confident driving Unisson vehicles which include large cars or vansEnergetic, compassionate,
