Point Care Pty Ltd is a registered NDIS provider and Child Safety service providercommitted to empowering individuals to live fulfilling, protected and independent lives.We provide Supported Independent Living (SIL), community access, youth support,residential care, child protection placements and complex care services across Queensland.We are seeking dedicated and compassionate Youth Workers to support children and youngpeople in residential homes, emergency placements, supported accommodation andcommunity-based programs.AbouttheRoleexperiencingsocial,emotional,behaviouralandotherchallenges.Youwillassistyoungpeopletodevelopessentiallifeskills,improvetheirwellbeing,engagewitheducationandcommunityservices,andworktowardstheirindividualgoals.Youwillcontributetotheassessment,development,implementationandreviewofindividualisedsupportplans,workingcollaborativelywithyoungpeople,colleagues,familiesKeyResponsibilitiesAssessthesocial,emotional,behaviouralanddevelopmentalneedsofchildrenandContributetothedevelopment,implementationandreviewofindividualisedcareandMonitorandevaluateyoungpeople'sprogressagainstsupportplanobjectives,documentingoutcomesandrecommendingadjustmentswhererequired.managepersonalchallenges,developresilienceandbuildpositiverelationships.Supportchildrenandyoungpeopletodevelopindependentlivingskills,includingEncourageparticipationineducation,vocationaltraining,employmentpreparationchildprotectionandcommunitysupportservices.Liaisewithfamilies,casemanagers,schools,governmentagenciesandotherserviceproviderstocoordinatesupportandpromotecontinuityofcare.Maintainaccuratecasenotes,assessmentrecords,progressreports,incidentreportsandotherrequireddocumentation.RespondappropriatelytoincidentsandcrisissituationsinaccordancewithindividualPromoteasafe,inclusiveandsupportiveenvironmentthatrespectstherights,dignityandindividualneedsofeveryyoungperson.Complywithapplicablechildprotection,workplacehealthandsafety,privacy,NDISRelevantqualificationinYouthWork,CommunityServices,Child,YouthandFamilyInterventionorarelatedfield.NDISWorkerScreeningClearance,whereapplicabletotherole.Strongwrittenandverbalcommunicationskills,includingtheabilitytomaintainAbilitytoworkcollaborativelywithyoungpeople,families,colleaguesandexternalserviceproviders.MandatoryTrainingSuccessfulapplicantswillberequiredtocompleterelevantorganisationaltraining,including:Medicationadministration,whererequired.
