Queensland Government's Metro South Palliative Care Service is seeking a compassionate Social Worker to deliver psychosocial assessment and support to adults in palliative care across community and inpatient settings, including Canossa and Wynnum Manly Palliative Care Units.You will work with a multidisciplinary team to manage a caseload, provide education to patients and families, and ensure ethical, evidence-based care while coordinating transitions between care settings.
