Support Plus 4 You Pty Ltd is seeking a proactive Community Worker to join our team in Heatherton, VIC.
You will meet clients, assess needs, and coordinate practical support to enhance wellbeing and independence.
You will develop education and training programs, refer clients to housing, health, employment and welfare services, and work with local groups to address community issues.
A minimum of two years' experience is required.
