Australian Unity Limited is seeking a Care Partner to guide customers through their care journey with compassion, coordinating services with families, clinical teams and external providers to deliver safe, person-centred care.
This full-time, hybrid role can be based from a nearby branch.
You will age well at home or in chosen settings, develop holistic health plans, and ensure protected discharge planning and ongoing support for customers in the community.
