BlueCare is seeking a Clinical Care Partner to coordinate complex clinical and care needs for clients across Arundel, Southport and surrounding areas. You will travel between sites, develop and review care plans, coordinate services, and monitor outcomes to achieve optimal care.
The role requires current AHPRA registration (RN or Allied Health), robust communication, time management, and the ability to work autonomously in the community. A driver’s license and vehicle are essential.
