You will work with clients to support independence and wellbeing, co-design care plans, and coordinate with the care team to achieve positive outcomes within budget and resource constraints.
Ideal candidates have a tertiary qualification in Aged Care or related field, solid communication, case management experience, knowledge of Home Care Standards, and a current driver licence.
