Bella Vista
to find a passionate Support Coordinator to join their friendly, close-knit team.
What you'll be doing
Visiting participants in the community and at home to build strong, trusting relationships
Coordinating supports and liaising with providers, families, allied health and other stakeholders
Supporting participants living with psychosocial disability and mental health challenges
Working with participants who have restrictive practices in place, and collaborating with behaviour support practitioners
Completing accurate, timely NDIS reporting, progress notes and plan review documentation
Monitoring plan budgets and advocating for participants at plan reviews
What you'll bring




Proven experience as a Support Coordinator within the NDIS space
Robust experience supporting participants with mental health needs
Solid understanding of restrictive practices and NDIS Quality and Safeguards Commission requirements
Confident NDIS report writing skills
A true self-starter attitude - organised, proactive and comfortable managing your own diary
Relevant qualification in Disability, Community Services, Mental Health or similar (highly regarded)
Current driver's licence and reliable vehicle
NDIS Worker Screening Check and Working With Children Check (or willingness to obtain)
