you'll undertake comprehensive aged care assessments in clients' homes, supporting older Australians to access services and maintain their independence. As a Clinical Assessor, you'll conduct Residential Aged Care Funding Assessments, working closely with residential aged care providers and residents.You'll:Conduct holistic, person-centred aged care assessmentsDevelop support plans focused on wellness, independence, and reablementIdentify appropriate supports, services, assistive technology, and community connectionsUtilise the Integrated Assessment Tool and My Aged Care systemsWork collaboratively with clients, families, healthcare professionals, and service providersManage your own workload while delivering high-quality outcomesThis is a rewarding role where you'll see the direct impact of your work every day.About YouYou're a qualified clinician who enjoys working independently while making a genuine difference in people's lives.You'll bring:Experience in aged care, community health, disability, health, or social services assessmentsStrong assessment, planning,
