Job Type: Part-time

cohealth is seeking a Community Health Nurse for a part-time ongoing role (0.4 FTE, 2 days: Monday and Wednesday) at our Fitzroy clinic in Melbourne. You will deliver culturally secure nursing care, assess and treat diverse clients, and develop tailored care plans with a multidisciplinary team to improve health outcomes.You will connect clients with services across sectors, provide health education, and participate in ongoing evaluation with our collaborative team.
