Clinical Care Manager - Community Aged Care
A well-established Brisbane community provider is now seeking an experienced RN to step into an exciting Clinical Care Manager role. This position offers a strong mix of clinical oversight, team support and service development within a collaborative, forward-thinking environment.
You’ll play a key role in providing high quality clinical practice and enhancing client outcomes, with a excellent balance of office and field work. Backed by supportive leadership and ongoing service growth, this is a fantastic opportunity to step into a role with real influence, progression and purpose.
Role & Benefits:
- $100k - $110k + super + salary packaging

- Manage an individual caseload of Support at Home clients
- Contribute to service improvements and high-quality care delivery

Key Duties:
- Conducting client assessments, care planning and ongoing reviews
- Managing Support at Home package budgets and funding utilisation
- Coordinating services across support workers, nursing and allied health
- Identifying changes in client care needs and arranging appropriate support
- Building strong relationships with clients, families and healthcare professionals
- Maintaining accurate client records and documentation
- Ensuring high-quality care in line with the Aged Care Quality Standards
About You:
- Current AHPRA registration (RN, EN or Allied Health)
- Previous experience in community aged care and case management
- Strong understanding of Support at Home funding, care planning and budgets
- Confident managing an individual client caseload
- Experience supporting clients with complex care needs
- Strong communication and relationship-building skills
- Organised, proactive and confident using client management systems
