GV Health is seeking a Clinical Care Coordinator to support Ambulatory Aged Care Programs (TCP, REST, GEM In The Home) in the West Hume Region. The part time role (64 hours per fortnight) coordinates care, advocates for clients and families, and works within an interdisciplinary team to deliver person-centred plans in acute, sub-acute and community settings.

Successful candidates will hold tertiary qualifications in nursing or allied health, current AHPRA registration where applicable, and
