Uniting is seeking an experienced Neighbourhood Care Partner to join our Home and Community Care team in Dubbo. You will manage client care plans, coordinate services, and advocate for client choice and dignity while ensuring funding is used appropriately.

You will travel between client homes and Uniting sites, building strong relationships with clients, families, and service providers, and contributing to quality improvement and secure work practices.
