ConneXtions is a not-for-profit organisation in Victoria dedicated to helping people with disabilities live inclusive lives. We seek a part time Support Coordinator in Wodonga to assist participants with NDIS planning, access to services, and coordination of supports.

You will collaborate with participants, families and providers to implement plans, monitor funding, and prepare for reviews, while upholding person-centred care and community engagement.
