Australian Unity Limited is seeking a Care Partner to guide customers through their care journey with compassion. You will work with families, clinical teams and external providers to create holistic health plans and coordinate services that enable aging well at home.
The role is permanent full-time with flexible schedule, a hybrid model (2 days WFH, 3 days in the Birtinya office) on the Sunshine Coast. Applicants should bring strong communication skills and experience in aged care.
