Cairns and Hinterland Hospital and Health Service is seeking an Advanced Health Worker to deliver culturally secure care in Hospital in the Home settings. You will provide direct patient care within the home environment across a broad local area.

You will conduct timely home visits, coordinate case management, and referrals while delivering health promotion and education sessions to individuals and families in the Aboriginal and Torres Strait Islander community.
