Sacred Heart Mission is seeking a Supportive Housing Case Manager to join the Homes First Inner South team in Melbourne. You will deliver culturally safe, three-year adaptable support packages to help participants sustain tenancies and access local community services.

You’ll work long-term with a small caseload, focusing on individualised goal planning, assertive outreach, and coordination with other agencies.
