As a Clinical Nurse Consultant - Frailty Intervention Community Service, you'll play a vital role in supporting older people with complex health needs to maintain their wellbeing, independence and quality of life. From advanced clinical assessment and care coordination to partnering with patients, families and health professionals, you'll help shape care that makes a lasting difference across the community.

Here's what will help you thrive in this role:
Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia.
Advanced clinical skills and experience caring for frail older people with complex or chronic health needs.
