Australian Unity is seeking a Care Partner in NSW to support customers living at home or in preferred settings. The role combines client visits, branch days in Queanbeyan/Moruya/Eurobodalla and two days of work from home, with a focus on person-centred, holistic care planning and coordination.
You will work with families, clinical teams and external providers to ensure protected, independent living and seamless transitions across services.
