Make a Difference in the Community We are looking for a compassionate and experienced Registered Nurse to join our Support at Home team and provide high-quality, person-centred care to older people living independently in their own homes. About the Role As a Registered Nurse, you will:

- Conduct comprehensive nursing assessments and develop individualised care plans.
- Provide clinical care including wound care, medication support and health monitoring.
- Monitor clients' health and respond appropriately to changes or clinical risks.
- Coordinate care with GPs, allied health professionals, families and support workers.
- Provide clinical guidance and support to care staff.
- Maintain accurate clinical documentation and ensure compliance with relevant aged care standards.
- Promote client choice, dignity, independence and quality of life.

About YouTo be successful, you will have:
- Current AHPRA registration as a Registered Nurse.
- Experience in aged care, community nursing, home care or a similar setting.
- Strong clinical assessment, communication and care planning skills.
- Ability to work independently and as part of a multidisciplinary team.




- A current driver's licence and reliable vehicle, where required.
- Current police check and relevant screening/clearances.
