About the role The Social Work Support Coordinator provides qualified social work support coordination for NDIS participants with complex disability, psychosocial, behavioural, and social support needs. The role focuses on person-centred planning, psychosocial assessment, risk-responsive coordination, and the delivery of ethically grounded support coordination that promotes safety, participation, and continuity of care.
Key responsibilities
- Work with participants, families/carers, and multidisciplinary providers to implement NDIS plans
- Coordinate funded supports
- Address barriers to access
- Strengthen functional and social outcomes
- Apply advanced clinical judgement and systems-based coordination where complexity requires it
