Clinical Care Coordinator (CCC)

Lead clinical and care teams. Oversee care planning, clinical governance, compliance, staff mentoring.

Key Responsibilities

- Lead clinical care team
- Oversee care plan development
- Ensure compliance with Quality Standards, Aged Care Act, AN-ACC
- Conduct clinical assessments and audits
- Manage and mentor RNs,



EENs, care staff
- Liaise with GPs, allied health, families
- Lead clinical governance
- Monitor documentation
- Manage clinical portfolios
- Participate in case conferences

Requirements

- AHPRA registration as RN
- Min 3 years post-grad experience in aged care
- Demonstrated senior clinical/leadership experience
- Strong knowledge of Quality Standards, AN-ACC
- Team leadership ability
- Excellent clinical assessment skills
- Clinical governance understanding
- Clinical information systems proficiency
- National Police Check
- Valid working rights
- Current flu vaccination
- COVID-19 vaccinations

Desirable

- Post-grad in aged care/gerontology
- Accreditation experience
- Palliative/dementia care knowledge
- Rostering/budgeting experience
