We have a highly engaged, diverse and collaborative workforce that is unified by our goal to help people live healthy and fulfilled lives.

About the Health Independence Program

The Health Independence Program (HIP) is funded by the Victorian Department of Health and is part of the Continuing Care Division. HIP provides multidisciplinary services that support people with health problems and other complex conditions to optimise their function and self-management skills so they can live independently and well at home and in the community.

The Health Independence Program includes many of the services that deliver health care to support the transition from hospital to home. These services can also prevent the need for a hospital presentation or stay, with people able to access them directly from the community.

This key leadership position is responsible for delivering high-quality, client-centred services while achieving activity, quality, workforce, and financial performance targets. The successful candidate will lead multidisciplinary teams across HIP specialist clinics and community rehabilitation programs, managing approximately 34 FTE and an annual budget of approximately $4.6 million.

Key Responsibilities:

- Lead and support multidisciplinary clinical teams to deliver safe,

high-quality care.
- Provide operational oversight of HIP Specialist Clinics and Community Rehabilitation Services.
- Drive performance against service targets, quality indicators and budget requirements.
- Lead workforce planning, recruitment, performance management and professional development.

About you

You will have excellent communication skills and a solid commitment to excellence in patient care. You’ll bring energy and drive to your work, and have a keen interest in developing your skill set.

To be successful in this role, you'll bring with you:

- Demonstrated experience leading large multidisciplinary clinical teams.

- Strong skills in program management, workforce management and budget oversight.

- Excellent communication, stakeholder engagement and problem-solving skills.

- Sound understanding of clinical governance across nursing and allied health disciplines.
