Australian Unity is seeking a Clinical Care Partner to lead and coordinate care for customers with complex needs across Mackay, Townsville, Cairns, Rockhampton, and Wide Bay. This permanent full time role offers hybrid work, flexibility, and a structured clinical governance framework.

You will mentor Care Partners, manage clinical risk, and support transitions of care while collaborating with multidisciplinary teams to improve customer outcomes.
