BlueCare, part of UnitingCare Queensland, seeks a Senior Personal Carer tosupport clients in Brisbane's Bulimba, Morningside, Coorparoo, Holland Park and surrounds.
You will conduct intake, create individualized care plans and deliver high-quality home care.You will visit clients in their homes to assist with daily activities and coordinate with the care team.
You will also manage records in CRM/CMS, ensure care plan adherence and participate in ongoing assessment of goals.
