NSW Health's Blacktown Hospital is seeking a Clinical Nurse Specialist Grade 2 - Discharge Coordinator to lead safe, timely discharge planning and continuity of care for patients with complex needs.
You will mentor nursing colleagues, collaborate across multidisciplinary teams, and drive quality improvement, clinical practice development and governance across the service.
A minimum of three years' full time clinical experience and AHPRA registration are required for this permanent, full-time
