BlueCare is seeking a Senior Personal Carer to deliver client-centered home and community care across Toowoomba North, Oakey and surrounds.
You'll conduct intake, screening, and develop personalised care plans, while supporting with daily living, transport, shopping and social activities in clients' homes.
You'll spend time on the road visiting clients, keeping CRM and CMS records up to date and collaborating with the care team to monitor progress.
