St Vincent's Private Hospital Brisbane in Melbourne VIC is seeking a agile Grade 3 Clinical Social Worker to support residents in our Residential Aged Care facilities. The RACFs covered include Berengarra, Prague House and Auburn House, with cross-campus service provision across SVHM sites.
You will deliver advanced psychosocial assessment, care planning and coordinated services, supported by a comprehensive professional development program and structured supervision for clinical decision
