Point Care Pty Ltd. is seeking dedicated Youth Workers to support children and young people in residential homes, emergency placements, supported accommodation and community-based programs across Queensland.
Responsibilities include assessing needs, developing and reviewing individualised care plans, monitoring progress and coordinating with families and service providers to ensure continuity of care.
Mandatory training will be provided, including medication administration where required.
