Uniting NSW.ACT seeks a dedicated Neighbourhood Care Partner to support older people in Dubbo and surrounding areas. You will deliver person-centred case management, develop care plans and coordinate in-home services to help clients live independently and with dignity.
The role involves budget management, advocating for client choice, and building solid links with families, carers and service providers. Regular travel between client homes and Uniting sites is expected.
