Bayside Health is seeking a proactive Social Worker to join our At Home Subacute team, delivering person-centred social work services across rehabilitation, geriatric care and palliative care in clients' homes and community settings.
You will conduct psychosocial assessments, provide counselling, and coordinate care with a multidisciplinary team, across home visits, telehealth and clinic appointments, to improve independence and quality of life.
