Think Mental Health is seeking a Mental Health Clinician to deliver in-reach services at Host sites across Canberra as part of the Mental Health Multidisciplinary Service (MHMDS). You will conduct intake, assessment and care coordination for consumers with mental health needs within a stepped-care framework.
The role requires collaboration with GPs, nurses and other allied health professionals, participation in case conferences and ongoing career development to improve service delivery.
