Right at Home Greater Logan is seeking a motivated and capable individual to join our team as a Care Partner, coordinating services and managing a portfolio of home care clients across the Greater Logan region.
We welcome applications from candidates with experience in care coordination, case management, nursing, aged care or other relevant community care roles. While previous experience in a similar position would be advantageous, we also encourage applications from individuals with transferable skills, strong organisational abilities and a genuine interest in progressing their career in community care.
We provide in-home care and support to older Australians (through the Support at Home program) and people living with disability across the Greater Logan and surrounding regions.
As a Care Partner, you'll take responsibility for managing your own portfolio of clients, coordinating services, overseeing care budgets and maintaining strong relationships with clients, families and care staff.
This is a varied role that requires sound judgement, strong organisational skills and the confidence to manage competing priorities. You'll need to be comfortable making decisions, resolving problems and taking ownership of your work.
You'll be joining an established office team with dedicated clinical, operational, scheduling and administrative support, allowing you to focus on delivering quality outcomes for your clients.
We're looking for someone who enjoys the challenges of community care, takes pride in their work and wants to contribute to a skilled, supportive and growing organisation.
Key Responsibilities
As a Care Partner, you will:
- Client Portfolio Management: Manage your own portfolio of clients,
building strong relationships and acting as the primary point of contact for clients, families and representatives.
- Care Planning & Coordination: Coordinate services, complete client onboarding and regular reviews, and ensure care arrangements continue to meet individual needs and preferences.
- Budget & Funding Management: Monitor client budgets, oversee service expenditure and ensure services are delivered within applicable funding guidelines.
- Client Advocacy & Wellbeing: Advocate for clients, monitor their ongoing wellbeing and work closely with our clinical team to identify and respond to changing care needs.
- Care Staff Support: Provide guidance and operational support to care staff, helping resolve service-related issues and maintain consistent, quality care.
- Stakeholder Relationships: Liaise with allied health professionals, external service providers and other stakeholders to coordinate services and achieve positive client outcomes.
- Compliance & Documentation: Maintain accurate client records, care plans and documentation in accordance with organisational procedures and relevant aged care and disability standards.
- Operational Support: Work collaboratively with our scheduling, clinical and management teams to resolve day-to-day challenges, contribute to continuous improvement and participate in a rotating after-hours on-call roster.
