The Salvation Army Australia is seeking a Care Coordinator (Enrolled Nurse) to provide coordinated clinical and operational support in the Bridge House Withdrawal Unit, Highgate, WA. This permanent part time (0.8 FTE) role supports admissions, discharge planning and seamless participant journeys within our Withdrawal Management Unit.
You will work closely with GPs, other health professionals and external providers, maintain accurate records, and contribute to quality improvement and service
